Send completed signed application with a check for the full amount to SOASOAS, PO Box 511, State College, PA 16804. Upon receipt, we send you a copy of the agreement and a parking hang tag for rearview mirror. Please e-mail any questions to staciewashell@comcast.net or fax (814) 238-1038.
